In this episode of Cafecito with Rosie, Dr. Ajoy Kumar discussed the benefits and kinds of vaccines available in the USA. Dr. Kumar was born in southern India and moved to the United States when he was five years old. His parents divorced, and he and his single mother moved from one place to another before settling in Washington, DC. He aspired to be a member of the CIA or the FBI, but after his first volunteer mission, he fell in love with the art of medicine. He then proceeded to study family medicine and eventually conducted several medical missions, delivered a lot of babies, and helped many patients. His advocacy aims to aid in the development of healthcare policy at both the state and national levels.Currently, he is taking care of senior patients at Dedicated Senior Medical Center in Largo, Florida.
Dr. Kumar said that 8 out of 10 people who died from COVID-19 were from the age group of 65 and above. People aged 32–39 years old have a 2x higher hospitalization rate and a 4x higher chance of dying from COVID-19. People over the age of 75 have a four-times higher hospitalization rate and a 220-fold increased risk of death. People who are 85 years old and above have a 13-times higher chance of hospitalization and a 630-times higher chance of dying. This alarming data is the reason why it is important to be educated about the vaccine.
Rosie asked what would happen if everyone got vaccinated, and then Dr. Ajoy stated that we could obtain herd immunity. Herd immunity occurs when a sufficient population of individuals get vaccinated and have resistance to the virus. This could drive down the rate of spread and how active the infection is in the population. At the same time, Dr. Kumar said that there would always be a population that can’t or won’t get vaccinated, but at least with herd immunity, we could indirectly protect them as well.
Dr. Kumar has educated the listeners on the types of vaccines. He said that there was no vaccine that was authorized to exactly prevent the virus, but that it would recognize a part of the virus instead. Typically, vaccines are made with mRNA, which contains a synthetically created spike of the virus that helps our immune system recognize it and eventually fight the virus. The Pfizer vaccine has a 95% efficiency. It consists of two shots, 21 days apart, for people aged 16 and up. Moderna is 94.1% effective for 18-year-olds and above, and should be taken two times, 28 days apart. There is also the Astrazenica vaccine, which is 63.09% effective and was developed on a non-viable virus and includes two shots. Johnson & Johnson has developed a one-shot and is 66% effective for mild illnesses and 86% effective for severe illnesses. Novavax is also available in the United Kingdom and South Africa. It is 89.3% effective and is a protein-based vaccine for 18-year-olds and above. In the United States, the FDA only granted emergency use authorization to Moderna, Pfizer, and Johnson & Johnson vaccines. Dr. Kumar also warned the public to never swap out or mix and match their vaccines.

Rita Barreto, the former chairperson of the Florida Commission on the Status of Women, is an empowered woman, a published author, an award-winning consultant, and the visionary and founder of Top Tier Leadership. She was born and raised in Miami, Florida. In her professional life, she spent 23 years in corporate America as a high-level director. Then, she took a leap and started her own business, Top Tier Leadership, 25 years ago.
In this episode, Rita shares her experiences as a 6-year commissioner. In 2020, she became the chairperson of the Florida Commission on the Status of Women. This Commission includes women of different races, socioeconomic backgrounds, professions, and geographic locations. Due to the diversity of the Commission they are able to come up with so many different solutions and approaches. She is proud to say that everyone in the commission didn’t skip a beat, even in the midst of the pandemic. They even held virtual events that enabled women to engage and be heard. They discovered that when they have a virtual event, more women attended than when they organized an in-person one. Rita developed and gathered more data concerning the status of women, which could be utilized as a tool for future project decision-making. Rita has also produced an annual report containing measurable data to aid in the development of policies to solve challenges that impact women and girls.
For many years, women have been suffering from discrimination. Women are not allowed to vote and don’t have the same rights as men. However, things changed 100 years ago as the suffragists fought for the rights of women. The suffragists pioneered equality and women’s empowerment.
Rosie and Rita talked about the 100 years of women’s suffrage in this episode. Rita reiterates that this is a significant event as it reflects where women have been. It celebrates women’s voices and the power to vote for someone to represent them in the government. Women’s suffrage enables us to have a voice and to make changes.
The Florida Commission on the Status of Women awards two major awards: Florida Women’s Hall of Fame and Spirit of the Community Award. While many may be familiar with the Women’s Hall of Fame award, many may not be familiar with the latter. Rita shared that the Spirit of the Community Award is a way of recognizing women who are currently volunteering in their communities with pure motivation. To nominate someone visit fcsw.net and complete an application and be sure to quantify what the nominee did in the community. A total of 12 women are selected. The Commission also has a video and essay contest for the youth related to women issues. Both boys and girls up to high school age can submit a video and essay. In 2020, the Commission purchased a kiosk that allows visitors to the State Capital to view the Women’s Hall of Fame awardees. Prior to 2020 visitors could only view this information by visiting the Women’s Hall of Fame wall at the Capital.
Towards the end, Rita suggests her favorite book, Leadership Challenge by James Kouze and Barry Posner. She loved this book as it is research-based and discussed the five characteristics of an admired leader: inspire a shared vision, model the way, challenging the process, enabling others to act, and encouraging the heart. She also advises everyone to pick one area to work on and once mastered move onto another characteristic.
In closing, it is important that we help each other rather than forcing our own way to the top. Have pure motives. The change starts with you because the only person you can change is you!

Nicole Valentin Alicea is a well-rounded marketing, branding, and advertising professional with superior business acumen. For almost 20 years, Nicole has successfully helped local, national and international companies, agencies, and nonprofits adapt to rapidly changing environments and achieve their business goals. Nicole shared that since hiring Lori she has continued to learn as much as she can about managing and supporting staff through conferences, podcasts, and real-life experiences. A business needs patience, teamwork, passion, skills, hard work, and perseverance. Employees are a company’s greatest investment. Striving and inspiring the employees is the greatest thing to do as a business owner.
Rosie shared that seventy percent of the economy of the United States is run by small businesses. As such, as a community we want small businesses to succeed. In that vein, Rosie asked Nicole to share a tip with other working mothers and Nicole shared that taking breaks throughout her day had led to increased productivity. What! Yes, taking a break is not a bad thing. It allows you to come back a bit sharper. She also suggested delegating tasks as much as you can is key; use your support system.

On this episode of Cafecito with Rosie we are talking about the topic of falling among the elderly with Dr. Nareesha Khan, an internal medicine specialist in Clearwater. She currently practices at Dedicated Senior Medical Center Clearwater LLC. Dr. Khan is board-certified in Internal Medicine. She received her medical degree from the University oOf South Florida College of Medicine and completed her residency at Orlando Regional Medical Center. Dr. Khan was born in Trinidad and Tobago and immigrated to Tampa at the age 4 and truly never left. She is living the American Dream. As long as she can remember she has worked with the underserved community.
At Dedicated Senior Medical Center, Dr. Khan shares that during this pandemic she has more and more patients with broken bones. If we don’t use it, we lose it. Not moving has its consequences.
Fall prevention is key. There are things to take note of to know when she and/or a caregiver should consider discussing fall prevention with a patient. How much effort is a patient using to get out of a chair? Is the patient experiencing back pain? In the home, check the carpet for any folds, which can cause the patient to trip, make sure all pathways are well light and clear, keep areas around the tub and sink dry, and keep cords tucked away. In the bathroom install grab bars and this makes it very easy to get in and out of the tub/shower.
Rosie mentions that often elderly people fall and once on the ground have no way of reaching a family member or friend. She asks what she can do as a neighbor of an elderly person to keep an eye on them. Dr. Khan mentions that if your neighbor has a routine that you are aware of and you don’t see them missing from that routine, give them a ring or knock on the door. If you have a family member’s phone number, give them a call. Due to Dedicated Senior Medical Centers hands on approach with patients, she often encourages her patients to give the Center’s phone number to neighbors. The Center treats patients as family and will aid in facilitating a check in.
Continue the conversation around fall prevention, Dr. Khan mentions yoga and Tai Chi for seniors are helpful and low impact exercises that aid in improving balance. She has seen marked improvements in patients who participate in these types of exercises. An advantage of the Medicare Advantage plans is access to Silver Sneakers. This online portal grants patients access to on demand yoga and Tai Chi along with many other virtual programs.

In this pandemic, seniors are at the top of the mortality rate. Isolation is very important and being away from the family is the hardest thing to do.
Volunteering with hospitals, mission trips, and health-related organizations while in college inspired Dr. George Wickramanayake to become a physician. “I find it rewarding to help people and build meaningful relationships at the same time,” he says. Having helped his grandma with her care, Dr. Wickramanayake says, “Treating seniors is personal to me. I understand the difficulties they face, and I will go the extra mile to help them understand their health and feel in control of their care.
In seniors’ care, isolation is the biggest factor, especially in this pandemic. It can cause a lot of depression and anxiety in patients. The Senior Medical Center is combating this by providing transportation to secure the food and necessities needs of seniors and bring them to the hospital whenever they need it. They also call their patients at least once a week to encourage them to participate in various online activities that are available to them.
It can be very frustrating that the vaccine is not easily accessible and can lead to what has been termed as the “vaccine blues”. Dr Wickramanayake warns that maintaining the social distancing and safety precautions is the best path forwards, along with maintaining constant communication with your doctor about the vaccine availability.
Senior Medical Center always looks after its patients and always practices safety protocols. There is no other clinic that will always go beyond and do anything to help our patients and we will find ways to communicate with their families in any form. Only the Senior Medical center is offering 24/7 services to our patients.

Steve Sepato is the most famous unfamous speaker in America. A speaker for 40 years in a corporate setting for management, meetings, and leadership. Steve teaches other speakers how to be engaging speakers and get rave reviews from the audience. He has 7 businesses and he teaches individuals how to be a good speaker and not boring.
Rosie Paulsen also shares her knowledge from a book she read, “clarity: the power you’ll generate from focusing on what’s most important.” Finding your clarity and knowing your passion is important for you to stand out in your expertise for you to succeed. Being a good speaker is not just about talking in front of people but also finding your passion as a listener.
Over the years, Steve learned that “love is a learning process” and there are five love languages. You need to understand your love language and also your loved ones. “Communication is vital to success.”
“If you are not clear on your purpose, your business will pull you out,” Rosie says. Opening new businesses will pull you and drag you from your loved ones especially if you are busy and you’re going to forget things because you’re focusing on your business.
Steve gives a piece of advice that for us to succeed, we should always open our day with meditation and be grateful for what we have, start imagining what we want, and find somebody who can help us with that imagining.
The show ends with a quote and advice from Rosie, “giving back is not just helping other people, it’s about encouraging them to dream too.” You will succeed if you are a blessing to your community. Life without a purpose is nothing. These days, we should know our purpose in life for us to live our life to the fullest and together with Dr. Cruz we will talk about Purpose.
Dr. Claribel Cruz. is one of the doctors at Senior Care East Tampa, Florida. She originated in Miami Florida, her father is Cuban and her mother is Puerto Rican. She is married and has 3 kids. She decided to be a Doctor because in her family it’s been in their DNA and she has loved to help people since she was a child.
The Doctors’ are dedicated to improving their patients’ physical and emotional well-being and focus on prevention and wellness, thereby increasing a patient’s quality of life. Dr. Cruz said that each doctor in their company has a small patient panel which allows them to create deep patient relationships and focus on their mission of prevention. Their goal is also to let the patient be more comfortable by talking with the doctors in a language they like.
The doctors at Sr. Care East Tampa encourage their patients by giving them the freedom to the patient to be healed, not just by taking medicine, but also by their willingness to improve their lives and change their behavior as well. During this time of pandemic senior medical centers also offer online appointments, and clinical tests and ensure that their staff is protected and also their patients by proper use of PPEs and preventive measures for the safety of their employees, patients, and everyone.

Dedicated Senior Medical Center is a business under ChenMed founded by James Chen and currently managed by the CEO Chris Chen. ChenMed is a family-owned and run company that started in Miami 30 years ago. The Chen family used to be impoverished. To give to the community, they are helping the underserved and underprivileged communities through accessible and complete medical facilities.
In this episode of Cafecito with Rosie, Dr. Nehal Gheewala, the National Director of Growth at ChenMed, shares the goal and mission of the institution. The DSMC is created to help senior patients with their medical needs. They ensure that every patient is treated as a family and a VIP. Dr. Gheewala asserts that to ensure they take care of patients properly, they offer complete and convenient services from medical assessments to transportation.
Dr. Gheewala said that the likelihood of people more than 65 years old being hospitalized increases. They have to be proactive in ensuring the health of every patient. On average, every patient calls their DSMC doctors and spends 240-260 minutes, if not more, every year. This interaction helps doctors to know every patient, their history, and their status. Doctors and staffs of DSMC do a weekly routined call to patients to know them better, just like we do for our families. It is possible as, unlike most hospitals around the Tampa area, they only have 400 patients per doctor.
The DSMC and its amenities have locations all over Tampa, as Dr. Gheewala mentioned. These locations are chosen based on the status of the community. Usually, it is where there are only a few stores, few means of transportation, or areas with a lack of doctors. This is to live their mission of helping impoverished citizens. Rosie was very impressed that the Dedicated Senior Medical Center partnered with local food banks to distribute food items to patients with no access to healthier options.

Changing your career path after finding your passion is a great way to live a fulfilling life. Turning your back from all the things that make you unhappy is fine after all; nothing is worth it if you are not happy.
Sergio Vanegas, the guest for the 8th episode of Cafecito with Rosie, has completely changed his life from his professional 9-5 job to running his filmmaking business alongside his brother. Sergio was born and raised in Bogota, Columbia, and moved to the US when he was young. He has a degree in finance and System Engineering. He used to work at an office desk but grew tired of it. He is a happily married man and a father to two sons.
After leaving his desk job, he learned every production aspect, photography, videography, and possible contacts. Currently, Sergio’s clients are typically those who want to grow their business. They range from start-ups to companies with marketing budgets. He ensures that all their clients could have ROI after their engagements.
In this episode, Rosie expresses how impressed she is with how Sergio never accepts less-paying customers. Sergio asserts that he exerts 150% effort and has a wide range of education, which is why he won’t settle for less. He stated that business owners should value themselves properly and believe in themselves.
The pandemic is a crisis for most businesses. However, Sergio’s business grew even more significant during this period. He reiterated how important adaptability is. He ventured in outsourcing and networking to ensure his business’ growth.
